To 6mos - in-take, assessment and case managementplease call for an phone interview.Eligibility Requirement:In order for you and your family to be found eligible, JTLS must verify that your family is in immediate need of temporary emergency shelter. JTLS will conduct an inquisition to determine whether this is a safe and appropriate place for you and your family to stay, even temporarily. To aid the inquiry, you should provide any documents that will help us understand why and how you now are homeless. -You must be homeless Verification of Homeless by HUD Guideline- Entering in JTLS Programs you must be drug and alcohol free- Must be employed or have an income, readily employable or going to school- Be willing to participate in the program and follow all program rules- Meet certain other criteria relating to substance addiction, criminal records, domestic violence, etc. Specific criteria may vary by programWhat To Bring:-You will need to have identification, such as a welfare ID card, green card, driver's license, passport/visa, or picture employment card. - If you do not have a picture ID, you can generally use a birth certificate, social security card, Medicaid card, identity card in the public assistance system, or a pay stub to prove your identity. - In order to apply at AFDC you need to verify that your household constitutes a family and must be verified. What To Expect:If your family is found eligible, you will be placed in temporary Emergency Shelter or Jordan's Transition Living Shelter. All clients will be expected to follow certain guidelines, which include:- Working closely with your caseworker or housing specialist to locate and view available housing or apartments;- Actively seeking permanent housing for you and your family by viewing available apartments several times per week;- Accepting a suitable apartment for you and your family when it is offered to you;-Following shelter guidelines that prohibit behavior that places other clients and staff at risk. Case Management:All clients must meet with their Case Manager for an assessment which identifies and prioritizes the clients' needs. The information gathered from this assessment along with the clients' input and future goals are used to develop a restoration plan for the client. Referrals are distributed and are necessary to assist the clients in overcoming the barriers of homelessness and unemployment. Individualized plan of services are created based on the clients' needs, strengths and future goals. There is an ongoing interaction with the client and the service provider to coordinate the activities. The Case Manager monitors the delivery of services to ensure that the client and the service providers are engaged in activities related to the original plan of action.
