The PATH program provide intensive, personalized outreach adn engagement services to persons who are homeless, or at risk of becoming homeless, andbr/ have serious mental illnesses or co-occurring substancebr/ use disorders. PATH is intended to provide time limited services and to be a bridge to traditional community and behavioral health services, either through regular MHMR  services or other appropriate community resources. Eligibility:  Homeless individuals with suspected mental illness or co-occurring substance abuse disorders. Hours:  Monday - Friday / 8:00 AM - 5:00 PM.
