Unity Care offers housing solutions that provide youth and young adults a safe and secure place to live as they focus on their emotional, educational, and employment goals. They integrate resources and support services to ensure the well-being of those they serve, and create a home-like environment where youth and families feel protected. THPP provides current foster youth ages 16-18 the necessary life skills to live independently upon exiting the child welfare system. Youth receive round-the-clock on-site support and supervision from the THPP staff and county social workers. They also connect them to Independent Living Programs in Santa Clara County.
